Rosenberg Self-Esteem Scale:

     Dr. Morris Rosenberg published the Rosenberg Self-Esteem Scale (RSES) in 1965. The questionnaire has 10 questions which are each answered on a four point scale from strongly disagree to strongly agree. This scale is used in up 53 different countries and is translated into various languages. The score of the scale ranges from 0-30. A score of less then 15 may indicate a low self-esteem.
